Consent Form Specifically for Varicose Veins RF Ablation

RF Ablation has displaced Standard Surgery except for really massive Varicose Veins. It is as effective and should be as durable, although very long term results are not available. This form is meant to be informative in addition to being used for consent.

Information regarding Angiography, Angioplasty and Stent

Read to understand the procedure that is necessary to unblock your arteries, it is also applicable to venous procedures including insertion of a Vena Cava  Filter, but because venous pressures are much lower there is less risk of substantial bleeding.

Open Surgery for Abdominal Aneurysm

This a patients personal experience of major abdominal surgery required for open rather than endovascular repair of Abdominal Aortic Aneurysm (AAA). This type of approach is now more common for Complex Anterior Spinal Reconstruction rather than AAA which in a large percentage can be managed with endo-Stentgrafts.
